• Setting : Free Standing Clinic
• Dates of coverage needed: ASAP (quick credentialing) to July 1
• Schedule : Monday Friday; 8a - 5p - will also cover at other locations
• Scope of Practice: Urgent Care, work related injuries, pre-employment physicals
• Experience : 2+ years of UC and Occ Med experience
• License and certification: Board Certified, OR State License, DEA, BLS, DOT Certification preferred
