Woodworth Community Health Center, located in Woodworth, Louisiana, is a member of the Access Health Louisiana Network. We are seeking a Family Nurse Practitioner or Physician Assistant to join our team.

The successful Family Nurse Practitioner/PA candidate will have or obtain an unrestricted license in the State of Louisiana. The salary range for this position is
$100,000-$115,000 per year. Salary isdependent on experience.

Position Description
Under the direction of the SVP of Medical Affairs and EVP of Operations, the Nurse Practitioner assumes the responsibility of routine exams. Utilizes skills in providing diagnosis and treatment of acute and chronic health problems along with providing care focused on health risk factor reduction. Ensures efficient and effective patient care through collaborative planning and problem-solving with center staff.
Minimum QualificationsEducation
- Current Louisiana APRN license.
- Must possess a Master's degree with a concentration in the respective advanced practice nursing specialty from an accredited college or university.
- Must possess a certification by a nationally recognized certifying body.
- Current CPR Certification
Experience
- At least one (1) year of experience in advanced practice nursing.
- Knowledge of scope of practice.

- Excellent analytical skills to assess and evaluate patient needs and develop and implement a care plan.

Physical and Mental Requirements- Stands and walks most of the day.
- Reads gauges, digital read outs, graphs and educational material, and patient charts in English (Visual acuity)
- Exerts up to 20 pounds of force frequently including pushing and/or pulling.
- Moves quickly from room to room and maneuvers in exam rooms and workspaces.
- Operates office and medical equipment.
- OSHA Category I: Duties performed routinely require exposure to blood, body fluid and tissue.
- Potential exposure to chemical hazards.
I: Essential Duties/Responsibilities (Position):Patient Relations- Achieves customer/patient satisfaction by greeting patients and proactively attending to their needs.
- Responds to patient requests in a timely and efficient manner.
- Maintains confidentiality of patient information at work and off duty.
- Provides health education to students, families, and/or community health practitioners and groups, within area of specialty.
Assessment- Performs medical examinations and evaluations and diagnoses, treatment, follow-up, consultations, and health education relevant to the patient's care (age specific).
- Obtains patient histories and develops patient care charts, ensuring accuracy and completeness.
