Period Of Performance: The period of performance will be a 12-month base from the date of award with 4 consecutive one-year options.

Services Provided: The Contractor shall provide Board Certified UROLOGY SURGERY Physician Services on site in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) and the COLUMBIA, VA HEALTH CARE SYSTEM, VAMC.
Place of Performance: Contractor shall furnish services at the COLUMBIA, VA HEALTH CARE SYSTEM, 6439 GARNERS FERRY ROAD, COLUMBIA, SC 29209.
QUALIFICATIONS:
License: The Contractor's physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia) when services are performed onsite on VA property.
All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contractor's physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
Board Certification: All Contractor's physician(s) shall be Board Certified by the American Board of UROLOGY SURGEONS and be currently certified in Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance.
2.1.5. Continuing Medical Education (CME)/ Certified Education Unit (CEU) Requirements: Contractor shall provide the COR copies of current CMEs as required or requested by the VAMC. Contractor's physician(s) registered or certified by national/medical associations shall continue to meet the minimum standards for CME to remain current. Contractor shall report CME hours to the credential's office for tracking. These documents are required for both privileging and re-privileging. Failure to provide shall result in loss of privileges for Contractor's physician(s).
VA HOURS OF OPERATION/SCHEDULING: The Physician(s) shall be available and present in clinic during normal Columbia, VA Health Care System clinic hours, Columbia, VA Health Care System which will be established, and may be revised, as deemed appropriate for patient care by the Chief of Staff. Currently, normal clinic hours are _Monday through Friday 0800-1630. Scheduled operating days are Monday, Wednesday, Thursday, from 0700- 1600. Contractor's Physician(s) shall report at a minimum of 20 (twenty) minutes prior.
Off-hours Coverage: The physician(s) available on call during all hours when the Columbia, VA Health Care System UROLOGY SURGERY Clinic is closed, including evenings, weekends, and holidays. The contract physician shall be required to carry a pager and or cellular phone and shall remain within a thirty (30) minute travel radium of the Columbia, VA Health Care System VAMC when on-call. Contractor shall not provide concurrent on call duties with any other medical facility while on call for Columbia, VA Health Care System. On-call will not be required while clinics or surgeries are in session; however, Contractor may be required to perform on call prior to and after clinic or surgery sessions. If the Contractor employee is to report back to Columbia, VA Health Care System VAMC; reimbursement shall be hourly for any services performed while at the VAMC. When a change in a patient's UROLOGY SURGERY condition becomes of a critical nature during non-administrative hours, the physician must immediately evaluate the patient document his/her evaluation of the events in the chart. Documentation of the physician's evaluation of these events in the chart. Documentation of the physician's evaluation of these events will be made in the chart within twenty-four (24) hours or within the next business day.
Physician(s) must always be available for phone consultations with VA residents and physicians and for non-critical patients, the physician must examine the patient the following workday. If the change in a patient's condition is of a non-critical nature or there is an urgent noncritical nature during non-administrative hours, the physician will be notified the next workday, so the physician can examine the patient. If the Contract Physician is to report back to Columbia, VA Health Care System VAMC, reimbursement shall be hourly at the same rate for services performed while at Columbia, VA Health Care System, VAMC.
All physicians who are On-call must be available within 15 minutes by phone and on-site within 30 minutes when medically required/indicated.
