Advanced Practice Nurse specializing in
geriatric nursing .
Key Responsibilities - Provide high-quality, compassionate care for patients in long-term care settings , focusing on geriatric nursing
- Collaborate with nursing staff, medical directors, and specialists to create comprehensive care plans tailored to geriatric patients.
- Accurately document visits in an electronic medical record (EMR)
- Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse .
Qualifications Required Qualifications: - Active Virginia Medical License and DEA License
- Minimum one year of clinical nursing experience
- Strong communication skills, self-motivated, adaptable, and a team player
Preferred Qualifications: - Nursing experience in ICU, Med/Surg, Emergency Medicine , or Hospitalist roles
- Experience with care coordination and working in long-term care settings, especially geriatric nursing
