RN Case Manager - Transitional Care

Full-Time / 36 Hours / Nights
Three 12-hour shifts (6p-6:30a) per week; weekday w/ rotating weekends
Summary:
Facilitates development, implementation, coordination, and evaluation of patient's individualized plan triggered by referral from nursing, physician, interdisciplinary rounds, and other care team members. Collaborates with patient, family, physician, and interdisciplinary team to assure care is provided from acute care through the continuum. Includes taking a leadership role with the interdisciplinary team to achieve actions and processes designed to ensure optimal clinical outcomes, coordination, and continuity of health care as patients transfer between different levels of care and promotes effective utilization of healthcare resources through monitoring the patient's hospitalization. Coordinates utilization review activities of assigned patients including reviewing and monitoring the utilization of health care services with the goal of maintaining quality, cost-effective, medically necessary care in the appropriate setting.
Minimum Qualifications:
1. Formal Education Required:
a. Graduate of an accredited BSN school of professional nursing or a school accepted for candidacy status from an accrediting body.
b. Registered Nurse, Licensed in the State of Ohio.
2. Experience & Training Required:
a. Two (2) years recent experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management that has provided a broad understanding in Case Management and Utilization Management concepts.
3. Other Skills, Competencies and Qualifications:
a. Demonstrates a high level of clinical competence that is recognized by peers.
b. Is knowledgeable of disease processes, treatment protocols and pharmacological management.
c. Demonstrated ability to communicate clearly and sensitively in writing and orally with physicians, patients and families, health care professionals, and others.
d. Ability to interact and collaborate with physicians and other health care professionals in a professional manner.
e. Ability to organize and coordinate the work efforts of others.
f. Ability to prioritize work and solve problems.
g. Assumes accountability for demonstrating behaviors consistent with the customer service policy.
h. Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity.
4. Level of Physical Demands:
a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.
