Job Description :
- Conduct discharge planning, chart review, and patient screening
- Coordinate care and collaborate with healthcare professionals
- Identify and mitigate potential barriers to patient care
- Exhibit strong organization and multitasking abilities
- Communicate effectively and efficiently within the healthcare team
- Wear navy blue scrubs or business casual attire, adhering to dress code policies
- Engage in a unit environment that values teamwork and collaboration
- Utilize computer, iPad, and work phone; experience with EMR systems such as Meditech and Midas is beneficial
- Participate in a unit with 34 beds supported by a team including 1 RN and 1 Social Worker for case management during the day
