$130,000-$140,000+ (based on experience)

Bonus potential: based on productivity, every 2 weeks.

Monday-Friday: 8am-5pm or create your own schedule!

Can start at 9:30am, 10am, 11am, etc.

No: on-call, weekends, or holidays.

Qualifications
n
Required:
n

Doctorate or Master's Degree in Physical Therapy.
n n- n
CA state PT license.
n n- n
2 years of experience as a PT.
n n- n
CPR Certification.
n n- n
Driver's License and reliable vehicle with insurance.
n n
n
Preferred:
n
n- n
Community or Hospice experience preferred. Open to other backgrounds as long as your understand Home Care and Hospice philosophy.
n n- n
PT lives in the Modesto, Oakdale, Jamestown, and Sonora, CA area to be able to treat patients in the Sonora, Jamestown, and Cold Springs, CA area.

The Physical Therapist will deliver comprehensive physical therapy services based on personalized care plans.

Comfortable going to patients' homes and caring and passionate, will take their time with patients and not rush them.

Patient population : seniors, post-surgical patients, stroke survivors, and individuals with cardiac conditions

Key responsibilities:

Assess patients' therapeutic needs.
n n- n
Evaluate physical capabilities (muscle strength, mobility).
n n- n
Direct appropriate treatment to enhance comfort and functionality.
n n- n
Educate patients and families on therapeutic devices.
n n- n
Report changes in patient conditions to physicians.
n n- n
Participate in discharge planning.
n n- n
Supervise Physical Therapist Assistants or Hospice Aides.
n n- n
Provide consultation to caregivers.
n n- n
Engage in interdisciplinary collaboration.
n n- n
Contribute to quality improvement initiatives within the organization.
n n
