As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:
n
n- Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
n- Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
n- Observe movements, conduct assessments, document progress and modify care plan as needed
n- Assist patient with exercises and hands-on therapy to manage pain and increase mobility
n- Educate patients on their plan of care, established goals, exercises and self-care
n- Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:
n
n- Graduate of an accredited Physical Therapist Program and active PT license in Kansas
n- CPR certification
n- Knowledge of state and federal home health regulations
n- Good clinical judgement, strong interpersonal skills, dexterity and compassion
