Job Description:
- Ensure completion of prescreen documentation attached upon submitting the application.
- Submit a resume and verify an active license.
- Collect and provide three professional references.
- Clear a county criminal background check for the past seven years for all counties lived and worked.
- Clear a federal criminal background check for over seven years.
- Complete a multi-state criminal background search for over seven years.
- Complete a nationwide sex offender search for over seven years.
- Pass HHS/OIG and GSA searches.
- Complete a Social Security Number trace.
- Provide proof of 1 Step PPD or Quantiferon, or proof of positive PPD and chest X-ray.
- Complete the SAM search.
- Clear the CORI Massachusetts background check.
- Complete 8 hours of dementia training.
