Job Description
The RN - Stroke Program Coordinator is responsible for the overall organization, development, planning, management, and evaluation of the Stroke Program. In conjunction with the director of clinical quality, coordinates overall Stroke education for staff and hospital departments. This position operates the Stroke Program within the approved budget and maximizes the utilization of personnel and departments through direct communication, critique, and continuing evaluation. This position participates in nursing and hospital-wide activities.
Responsibilities
  • In conjunction with managers and directors, coordinates and develops aspects of the Stroke Program.
  • Maintains current knowledge of stroke care through a review of the literature, and attendance at regional, state, and national conferences. Assures accuracy of case finding methodologies for the Stroke Program to identify stroke patients entering the system and follows their progress.
  • Responsible for the direct oversight of stroke data. Oversees the accuracy, validity and reliability of the stroke database/registry; gathers statistics and data, identifies trends and provides follow-up through quarterly reports per Joint Commission requirements.
  • In accordance with the Hospital Quality Improvement Program, develops, implements and maintains a Quality Improvement Program for the Stroke Program.
  • Monitors the collection of data via the stroke registries alongside the Quality Data Analyst for concurrent and retrospective review and forwards conclusions to the Director of Clinical Quality.
  • Makes recommendations for improvement to appropriate personnel based on data analysis trends.
  • Responsible for the establishment and coordination of ten (10) multidisciplinary conferences on an annual basis. Coordination includes: Invitations, announcements, sign-in sheets, and documentation of minutes.
  • Assists in development and implementation of unit policies and procedures specific to stroke care and maintains annual review of the Policy and Procedure Manual.
  • Evaluates nursing care of the stroke patients and collaborates with nursing and other hospital managers/leaders/staff and the Director of Clinical Quality, to establish and/or update clinical procedures and standards of care.
  • Participates in establishing objectives, standards of care, and policies for the stroke program through the development of stroke protocols.
  • Assists Case Management for the stroke patient to include clinical assessment and evaluation, acts as physician and family liaison, performs utilization review and discharge planning when necessary.
  • Collaborates with patient care staff and unit managers to identify learning needs, define expectations of learner, and determine the most appropriate effective approach to meet identified needs according to the unit type.
  • Functions as a role model to nursing staff.
  • Participates directly in selected clinical and educational activities when necessary.
  • Participates in the continuing education and training of stroke nurses, and other hospital providers.
  • Develops an educational plan which is evaluated annually to ensure that all required education is met.
  • Utilizes a flexible schedule to provide visibility on all three shifts by meeting staff and/or patient educational needs when necessary.
  • Serves as a consultant in patient/family educational issues to include consideration of age specific criteria when necessary.
  • Serves as a community liaison in areas of Stroke education.
  • Provides community stroke prevention and outreach education programs.
  • Maintains current knowledge/certification/licensure.
  • Pursues professional growth and development.
  • Assist in other duties as assigned to support the operational needs of the department and organization.
  • May be required to remain on campus immediately before, during, and after severe weather and/or disasters.
