JOB SUMMARY The Hospice Spiritual Caregiver is responsible for the provision of spiritual care services to patients and families/caregivers of Sheridan Memorial Hospital Hospice, either directly or through coordination of care with hospice team members.
ESSENTIAL DUTIES/RESPONSIBILITIES - Performs an assessment of the spiritual needs of the patient and family within 5 days of the patient's admission to hospice services.
- If the patient or caregiver declines the spiritual caregiver visits, the spiritual caregiver will serve as a resource for other interdisciplinary team members assessing the patient/caregiver's spiritual needs.
- Spiritual assessment shall include but not be limited to the identification of any religious affiliation the patient and family may have and identify the nature and scope of any spiritual concerns or needs.
- Participates in the development of an interdisciplinary plan of care to meet identified spiritual needs.
- Provides direct spiritual care to patients, families, and caregivers.
- Documents spiritual care services utilization by the patients, families, and caregivers.
- Participates in interdisciplinary team meetings.
- Acts as a professional resource for consultation, education, and support to the interdisciplinary team on spiritual care.
- Conducts funeral or memorial services for patients as needed and requested.
- Assists the Interdisciplinary team in providing an annual memorial service to meet the needs of families, personnel, and volunteers in the assigned office.
- Collaborates with patient's personal spiritual or community clergy and spiritual counselors as well as the interdisciplinary team.
- Coordinates bereavement care with the bereavement coordinator for the caregiver and family members for a period of 12 months following the death of the patient.
- Reaches across faith group boundaries and does not proselytize. Acting on behalf of the patient's religious affiliation seeks to protect the patient from being confronted by other, unwelcome forms of spiritual intrusion.
- Respects the religious preference of the patient.
- Performs other duties where assigned.

POSITION QUALIFICATIONS
Education/Experience/Licenses - Bachelor's Degree from a CHEA accredited (or international equivalent) institution in a content area relevant to chaplaincy.
- Acceptable concentrations would include but would not be limited to theology, study of sacred texts, medical or professional ethics, psychology, sociology, family systems, counseling, social work, nursing, world religions and belief systems, organizational development, gerontology, communication, and the relationship of spirituality and health.
- Completion of hospice volunteer training program within orientation period.
- Completion of Spiritual Care Generalist Certificate or Chaplain Credentialing (CC) within one year of hire.
- Completion of Self-Guided Palliative Care Courses within two years of hire.
- Criminal and Central Registry background check is required upon hire.
- De-escalation training within 1 year of hire.
Additional Skills - Comfortable in an inter-denominational setting.
- Excellent interpersonal skills to relate effectively to staff, patients, caregivers and significant others.
- Emotional and mental maturity.
- Valid driver's license

- Able to communicate effectively in English, both verbally and in writing.
Specific demands not listed: Possible exposure to blood and or body fluids / infectious disease / hazardous waste requiring the use of Personal Protective Equipment. Exposure to odorous chemicals / specimens and Latex products.
