Join our team as a dedicated Physical Therapist (PT) on a per diem basis, where your skills and commitment will significantly impact patients' lives. We are seeking a compassionate and skilled professional to provide therapeutic care aimed at enhancing mobility, reducing pain, and restoring function through tailored treatment plans.
Key Responsibilities:
- Review resident information to evaluate its impact on assessments and treatments.
- Conduct comprehensive functional assessments using standardized procedures.
- Develop and implement personalized treatment plans with realistic functional goals.
- Ensure compliance with evaluation, treatment, and documentation standards.
- Supervise and instruct therapy assistants during treatment procedures.
- Document treatment progress and revise plans as necessary.
- Educate residents and caregivers on exercises and procedures for home continuation.
- Collaborate with nursing staff and families to maximize residents' functional abilities.
- Participate in team meetings and quality improvement initiatives.
- Stay informed of community resources and assist in referral processes post-discharge.
Qualifications: - At least one (1) year of experience as a PT, Home Health experience preferred
- Valid California PT license required
- Current CPR certification required
- Current California driver's license, good driving record, and reliable transportation
- Located in Southern California
Physical Requirements: - Ability to stand for prolonged periods.
- Capability to push or pull with a force of up to 35 lbs.
- Ability to lift up to 50 lbs independently and over 50 lbs with assistance.
- Capacity to stoop, reach, and maintain balance on various surfaces.
