Shift : 8P – 4.30p, Day Shift
Guaranteed Hours : 40 Hours in a Week
Weekly Pay Range: $2100.00 – $2130.00
Responsibilities :
- Assess patients’ medical, emotional, and social needs to create individualized care plans.
- Coordinate and oversee care services across healthcare settings.
- Monitor patient progress and modify care plans as needed.
- Educate patients and families on care plans, treatments, and resources.
- Document all case management activities accurately and timely.
- Advocate for patient needs and facilitate resource utilization.
Qualifications:
- Active RN License for FL or Compact license is required.
- Case management experience is required
