    Job Requirements
  • American Heart Association certification required.
  • License/Certificate Verification within 30 days of start.
  • Fit Test required.
  • 9 Panel Drug Screen within 30 days of start.
  • Trinity Code of Conduct Form on file.
  • Standard I9 completed by the agency.
  • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement on file.
  • Trinity Integrity and Audit Services Form.
  • Specialty exam completion within a year.
  • Trinity Color Vision test required.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
  • COVID-19 Vaccination proof or exemption (not valid for CA or IL).
  • MMR documentation required.
  • Hepatitis B series vaccination required.
  • Influenza Vaccine proof or exemption (not valid for CA or IL) within flu season dates.
  • Varicella documentation required.
  • Tdap documentation required.

    Schedule Information
  • Shift: Friday, Saturday, Sunday from 7 PM to 7:30 AM
