The Blended Case Manager (BCM) will promote resiliency and recovery for children, adolescents, their families and adults by accessing, coordination and mentoring community resources and services.
Requirements:
~ Proof of education.
~Act 33/34 clearances newer than 12 months.
~FBI clearance.
~Valid Pa driver’s license.
~ Reliable transportation.
~Proof of PA insurance.
~Physical health examination- within one year.
~Effectively manage a case load of 25-30.
~Share the responsibility of 24/7 on-call coverage with the other case managers.
~Attendance at weekly staffing and supervision meetings.
~Participation in annual out service training.
~After 90 days with caseload of 25-30, average production of 100 units (25hrs) per week.
Essential Functions:
~ Advocate for the consumer and family.
~Minimum of one face to face contact each month.
~Development of a strength based initial service plan within 30 days from admission.
~Development of an after-care plan from date of admission.
~Review and update service plan every 6 months at minimum.
~Maintain consumers’ chart.
~Coordinate formal and informal services.
~Attend and participate in all consumers related meetings.
~Promote the use of assessment and inventory tools.
~Measure and report outcomes.
Knowledge, Skills, and Abilities:
~Knowledge of the formal and informal supports of the consumers community.
~Knowledge of the unique needs of children and their service system.
~ Knowledge of the unique needs of transition age consumer and the adult system.
~Knowledge of the adult “WRAP” process.
~Skilled in writing measurable service plans.
~Knowledge of the person-centered planning model.
~Use best practices approaches.
~Know and apply CASSP and CSP Principles.
~Knowledge of psychiatric diagnoses and medications.
~Effectively manage consumers with behavioral and mental health issues.
~Ability to remain calm and think clearly in crisis situations.
~Communicate effectively and professionally-verbal and written.
~Establish and maintain positive professional relationships.
