Long-Term Service Care Manager

Location: Duval County, FL

About the Role:

Join us as an LTSS Service Care Manager , where you'll manage a caseload of healthcare members with long-term care needs. This role involves 80% travel within the assigned region and offers a mix of remote work and in-person visits. A valid driver's license is required.

Key Responsibilities:

• Conduct member assessments to develop and monitor care plans.
• Authorize and coordinate services to ensure seamless care delivery.
• Collaborate with caregivers, providers, and discharge planners to ensure effective care transitions.
• Provide support for complaints and appeals.

Qualifications:

• Education: Bachelor’s degree, RN license with 2+ years of care management experience, LPN with 4+ years, or 6+ years of care management experience.
• Preferred Experience: Home health, discharge planning, or long-term care experience.
• Skills: Microsoft Office, electronic medical health records, and long-term care expertise. Bilingual in Spanish is a plus!
