Licensed Practical Nurse (LPN) Job Description The Licensed Practical Nurse (LPN) is a vital member of the healthcare team at (Assisted Living Facility Name). Responsible for providing direct patient care, the LPN collaborates with Registered Nurses (RNs) and Certified Nursing Assistants (CNAs) to ensure the well-being and comfort of residents.
Responsibilities - Resident Care:
- Administer medications and treatments as ordered by physicians.
- Monitor resident health status, including vital signs, and report any changes to the RN.
- Assist with resident activities of daily living (ADLs) as needed.
- Provide wound care, ostomy care, and other treatments as ordered.
- Participate in resident care planning and implementation.
- Medication Administration:
- Prepare, administer, and document medications according to facility policies and procedures.
- Ensure medication accuracy and safety.
- Monitor for medication side effects and report any adverse reactions.
- Documentation:
- Maintain accurate and up-to-date resident records.
- Complete incident reports as required.
- Document medication administration, treatments, and resident assessments.
- Team Collaboration:
- Collaborate with RNs, CNAs, and other healthcare professionals to provide comprehensive resident care.
- Delegate tasks to CNAs as appropriate.
- Participate in staff meetings and in-service education.
- Emergency Response:
- Respond to resident emergencies in accordance with facility protocols.
- Administer emergency care as needed.
Qualifications - Current LPN licensure in the state of Ohio.
- Experience in long-term care or assisted living preferred.
- Strong clinical assessment and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a team.
- Compassionate and empathetic demeanor.
- Current CPR certification.
Working Conditions - Fast-paced environment with frequent interruptions.
- Ability to lift up to 50 pounds.
- Exposure to infectious diseases.
- Irregular work hours may be required.
