Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely patient transitions to appropriate care levels, preventing unnecessary admissions or readmissions. Your role will encompass effective communication and resource coordination to facilitate care across the continuum. The goal is to achieve optimal health, access to care, and appropriate resource utilization, balanced with patient self-determination. Collaborate with patients, families, physicians, interdisciplinary teams, and various departments to ensure efficient care management. Job Accountabilities: Conduct patient initial and continued assessments, gathering medical, psychosocial, and financial information. Lead the healthcare team in developing comprehensive, cost-effective care coordination plans. Identify and refer quality and risk management concerns for patient safety reporting. Oversee Case Management Assistants for post-acute care service preferences. Utilization Management: Review medical records to ensure patients meet level of care requirements. Collaborate with attending physicians for necessary documentation. Expedite transition planning for patients no longer requiring acute care. Monitor length of stay and outliers needing additional resources. Participate in patient rounds and collaborate with interdisciplinary teams for timely transitions. Care Coordination/Care Transitions: Formulate transition plans with input from patients, families, and healthcare teams. Screen 30-day readmissions and create effective transition plans. Identify barriers to care progression and work with teams to resolve them. Ensure timely transitions to lower levels of care and compliance with regulatory requirements.
