DUTIES and RESPONSIBILITIES:

• Provides excellent patient care that is focused on wellness, prevention of illness, and acute problem and chronic disease management.
• Identifies patient normal and abnormal findings through assessment of the physical, psychosocial, and developmental status of individuals and families, which is performed through independent and interdependent actions including health history, physical exam, laboratory screening, diagnostic tests, referrals, and collaboration with other medical providers to develop appropriate and quality diagnostic treatment.

• Licensed as an Advanced Registered Nurse Practitioner or Physician Assistant in the State of New Hampshire.
• Graduate of master's degree level nurse practitioner or physician assistant program.
