Job DescriptionTHIS POSITION REQUIRES A 90-DAY IN-PERSON ORIENTATION/TRAINING/PROBATIONARY PERIOD THEN MAY BE ELIGIBLE FOR A HYBRID-REMOTE SCHEDULE. CANDIDATE MUST LIVE IN THE ABQ AREA OR WITHIN COMMUTING DISTANCE. JOB CATEGORY: CARE COORDINATION / CASE MANAGEMENT / SOCIAL WORK / NURSEPOSITION SUMMARY: Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost-effective health outcomes.QualificationsREQUIREMENTSEDUCATION AND/OR EXPERIENCE: Bachelor's or Master's degree in social work, nursing or other healthcare profession; AND minimum two years' healthcare experience. Self-starter with strong verbal and written communication skills. Comfort with change and quality improvement processes. Experience with Electronic Health Records strongly preferred. Experience with Motivational Interviewing strongly preferred. Knowledge of New Mexico and local community resources strongly preferred. Bilingual (English / Spanish) or (English / Navajo) preferred.CERTIFICATES, LICENSES, REGISTRATIONS: Must obtain by date of hire and maintain current throughout employment: o Nursing license in New Mexico (e.g., LPN or RN); OR o Independent behavioral health license in New Mexico (e.g., LCSW, LISW, LPCC, LMFT, LCP or LPAT). Certified Case Manager (CCM) preferred.SCREENING REQUIREMENTS: This position requires successful completion of an initial post-offer of employment: According to PMS Human Resources Policy 205.006, "Healthcare Personnel Testing & Immunization Requirements," this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.
