Job Description:
Full-time inpatient RN Case Manager role with a top hospital in Fairfield county, CT, just over the border from NY. The schedule is Monday through Friday, 8am to 4pm, with 2 weekends per month plus holiday rotation required. The RN Case Manager is responsible for overseeing the implementation of patients' healthcare services. Responsibilities:Facilitates the care of patients to ensure quality outcomes and appropriate utilization of health care resources.Collaborate with a multi-disciplinary team in the planning, implementation and evaluation of medical and nursing careProvide care coordination in order to close gaps in carePerforms all job duties and responsibilities in a courteous and customer-focused manner Requirements:Registered Nurse license5+ years of strong clinical experience in an acute care setting.Additional experience in home care, long term care and case management is preferred
