Judge Healthcare is seeking a Case Coordinator in Cherry Hill, NJ for a fantastic client!


Location: Cherry Hill, NJ 08034

Hours: Monday-Friday 8:00 AM-5:00 PM

Schedule: Monday to Friday, Hybrid (3x a week on-site)

Length of Contract: Ongoing Contract (W2, Weekly Pay, Benefits/401K available)

Pay : Based on experience


Responsibilities:

  • Case Management: Manage access to services, family advocacy, and funding for assigned caseloads, reflecting company's core values.
  • Annual Plan Coordination: Schedule and coordinate annual plan meetings, ensuring inclusion of all care team members.
  • Documentation Preparation: Prepare and distribute documentation for annual plans to relevant parties promptly.
  • Service and Funding Authorization: Ensure annual plans accurately capture needed services and secure funding authorizations.
  • Eligibility and Benefits Checks: Facilitate eligibility checks and manage re-determination or restoration of benefits.
  • Funding for Medical Equipment: Assist in securing funding authorizations for durable medical equipment and adaptive devices.
  • Service Transition Support: Coordinate transfer meetings and documentation for seamless service transitions.
  • Contact Information Maintenance: Maintain accurate contact information for individuals and their care teams in Salesforce.
  • Electronic File Management: Oversee electronic files for the assigned caseload.
  • Legal Documentation: Ensure guardianship and Power of Attorney documents are on file.
  • Insurance Issue Resolution: Assist with resolving medical insurance issues for families and residential care teams.
  • Admission Coordination: Attend pre-admission and admissions meetings to understand service needs.
  • Discharge Planning: Prepare discharge letters and summaries, coordinating the discharge planning process.
  • Regulation Compliance: Maintain a clear understanding of funding regulations relevant to the caseload.


Requirements :

  • Bachelor's degree required
  • Minimum 2 years in case management and family advocacy.
  • Nurse Case Manager or Social Work experience preferred.
