Job Description

Are you ready to lead with impact? We’re seeking a dynamic Director of Case Management in Lima, OH , to oversee and optimize clinical care coordination programs. This role offers the opportunity to drive quality patient outcomes while ensuring cost-effective care across the continuum.


Key Responsibilities:

  • Provide strategic direction for clinical care coordination, discharge planning, and case management services.
  • Collaborate with interdisciplinary teams to improve care transitions, reduce readmissions, and enhance throughput.
  • Manage departmental resources, budgeting, and quality control while ensuring regulatory compliance.
  • Serve as a resource to senior leadership, staff, and interdisciplinary teams, promoting innovation and evidence-based practices.
  • Facilitate performance improvement processes and align departmental operations with organizational goals.


Qualifications:

  • Experience:
  • 5+ years in case management, discharge planning, or cost/quality management.
  • 3+ years in a hospital clinical role.
  • Supervisory/management experience required.
  • BSN required; Master's degree preferred.
  • Active Ohio RN License (Required).
  • BLS (AHA) Certification (Required).
