Job Title : Director of Case Management
Shift : 8:00 AM - 5:00 PM (40 Hours per week)
Location: Lima, OH, 45804
Travel Pay Breakdown: Weekly Gross - $3632 ($59.65/hr. on W2 + $1246 Weekly Non-Taxed)
Local Hourly Pay: $80 - $85 on W2
Job Overview :
The Director of Case Management, under the direction of the Chief Nursing Officer, leads the clinical care coordination program, ensuring quality and cost-effective patient outcomes across the continuum of care (from acute inpatient care to chronic disease management and preventative care). This role involves overseeing a team responsible for care coordination, negotiation, and procurement of patient care services.
Key Responsibilities :
- Provide overall direction and management of the Case Management department, ensuring human, fiscal, and physical/material resource management.
- Monitor and improve care transitions, reduce readmissions, and enhance patient satisfaction.
- Collaborate with interdisciplinary teams and other departments to improve patient outcomes, reduce risks, and drive performance improvement.
- Manage budgeting, quality control, and oversight of case management and discharge planning services.
- Serve as a resource to senior leadership on care coordination and regulatory standards.
- Promote team building, innovation, and processes that enhance the work environment.
- Implement evidence-based practices to improve patient care and clinical outcomes.
- Align operational plans with the Health System's strategic objectives and ensure high levels of professional practice.
Qualifications :
- Minimum 5 years of experience in case management, discharge planning, or other cost/quality management programs.
- Minimum 3 years of experience in a hospital clinical role.
- Supervisory/management experience required.
- BSN required, Master's degree preferred.
- Ohio RN License required.
- BLS (AHA) certification required.
