Salary begins at $158,378 + per year depending on experience based on a 1.0 FTE.

***$25,000 Sign-On Bonus & $25,000 Retention Bonus After 5 Years of Service***


The purpose of an Optometrist is to provide comprehensive eye care, completing exams to detect defects in vision, signs of injury, ocular diseases or abnormality or other general problems such as high blood pressure or diabetes, and engage patients in treatment plans that include tools to support their eyesight through both in-person and telehealth patient care.


Job Summary:

Optometrists perform routine and comprehensive eye exams, they assess an individual's vision and provide prescriptions for glasses and contacts; diagnose, treat, and manage eye injuries; detect disease in the eye; and provide services to rehabilitate eye conditions through low vision rehabilitation and vision therapy.


Essential Functions of the Job:

  • Conduct routine and comprehensive vision and eye exams to assess vision, evaluate symptoms, and identify disease and injury to diagnose and treat conditions.
  • Prescribe corrective lenses and provide services to include routine and specialty fittings.
  • Prescribe medications to treat ocular disease and provide comprehensive instructions for administration.
  • Interpret test and exam results based on scientific knowledge and the patient’s health history.
  • Reach an informed diagnosis based on scientific knowledge and the patient’s health history.
  • Provide education to patients and families on wellness, prevention, and treatment plans to promote vision and eye health.
  • Collaborate with family medicine and other providers to deliver high quality and integrated care.
  • Refer to specialty services as indicated.
  • Collaborate with program leadership to plan and participate in training and skills development for clinical support staff.
  • Participate in quality improvement initiatives.
  • Keep patient records including diagnosis, treatment and other significant information.
  • Cultivate a climate of trust and compassion for patients and clinical team.
  • Remain up-to-speed with developments and best practices in optometry ad ocular health.
  • Plans and administers therapeutic treatment by telephonic/ telehealth methodologies as indicated by patient needs and state guidelines.
  • Appropriate use of PPE/infection control protocols when providing treatment to patients.
  • Other duties as assigned.



  • Education: Doctor of Optometry (OD) degree
  • Licensure/Certification: Current, unrestricted license to practice in the State of Wisconsin required.
  • Experience with Systems, Computer Applications/Software : EPIC or related EHR experience.
