Location: Hybrid role (field-based work within the MetroPlusHealth service area)
Are you a passionate and skilled Licensed Master Social Worker (LMSW) ready to make a real impact in people’s lives? Join us in empowering members to transition toward better health and well-being.
What You’ll Do:
?? Assess and address members’ clinical, psychosocial, and environmental needs.
?? Collaborate with members, caregivers, and providers to create personalized care plans.
?? Support members through transitions from shelters to permanent housing and beyond.
?? Advocate for members to maximize access to essential resources.
?? Work with clients experiencing homelessness, addictions, mental health challenges, and more.
?? Travel within the MetroPlusHealth service area to connect with members, providers, and community agencies (1 day/week in the field).
What You Bring:
? Licensed Master of Social Work (LMSW) with valid NYSED certification.
? At least 2 year of LMSW experience and 3 years in care management, social work, or case management.
? Strong skills in problem-solving, time management, and communication (written & verbal).
? Proficiency in Microsoft Office and web-based applications.
? Experience working in behavioral health, housing rights, and community outreach.
? Ability to read and interpret medical records, claims data, and more.
? Bilingual (English/Spanish) preferred.
