Description
Under the leadership and direction of the Ruth's Place Manager, the Case Manager/ Intake Specialist will provide services to homeless transition aged youth (18-24 years old). The Ruth's Place Case Manager/Intake Specialist will determine eligibility of all clients enrolling with Coalition for Responsible Community Development for services. The Case Manager/ Intake Specialist will be providing case management services to new enrolled clients as needed, answering income phone calls, and providing adequate resources to prospective youths inquiring for homelessness services.
ESSENTIAL DUTIES & RESPONSIBILITIES
•Certify client documentation as prescribed by LASHA regulations, organize client files, maintain case notes.
•Conduct orientations and information sessions to small and large groups.
•Coordinate presentations and enrollment of participants with mandatory partners and cross functional teams.
•Review enrollment packets for completeness and accuracy.
•Determine accurate participant enrollment into the correct and appropriate programs.
•Conduct outreach and marketing efforts to promote Ruth's Place services and programs to increase enrollments. Activities include but are not limited to job fairs, recruitments, community events, places of wordship services or events, partner agencies events, PTAs, school events and more.
•Oversee the intake office, provide outreach, resources, intake, and assessment of clients to determine client needs and provide referral services.
•Attend meetings and trainings to improve skills and knowledge of best practices in Permanent Supportive Housing and client related services.
•Answering inbound calls in timely and courtesy manner
•Knowledgeable and familiar with Homeless TAY Population
•Provide exceptional customer service to new and existing consumers when needed.
•Enter client demographic information to HMIS and Salesforce databased system in place as needed.
•Maintain contact with partner agencies, when necessary, by email and or follow up with phone calls.
•In coordination with the Ruth's Place team schedule in person assessments and coordinate admission as required
•Expect to work some weekends and flexible to cover other position at the Ruth's Place when it is short on staff.
•In coordination with the Ruth's Place Manager maintain department and program presentation for information sessions, community outreach activities and inter- department outreach.
•In coordination with the Ruth's Place Manager develop and organize enrollment packets with current and updated forms mandated by LAHSA
•Uphold CRCD's Mission Statement and 5 Year Strategic Plan
•Attends and participates in Care Coordination Meetings for SPA 6 and various other community meetings.
•Represent CRCD/Ruth's Place at community resource events as needed.
•Comfortable working independently and with multidisciplinary team in a fast-paced environment
•Provide linkage to youth to resources such as education, employment. benefits, mental health, crisis bridge housing, transitional housing
•Formulate HSP that identify the youth needs and goals to help them decrease barriers of homeless and obtain housing, education, employment etc.
•Enter weekly case notes in HMIS database to document progress in the areas supporting the youth rapid transition to permanent housing.
•Complete Monthly Updates with youth to assess progress towards achieving their goals defined in the HSP.
•Assist youth with obtaining supporting documents and become document ready for housing.
•Comfort with working TAY homeless or at risk of being homeless population.
•Compassion for TAY, ability to manage time efficiently, prioritize tasks, demonstrate sound judgement, give attention to details, flexible, self-driven and motivated.
Requirements
•Knowledge of database management and quality control processes
•Reliable, flexible team player who works well with minimal supervision, who has a good work ethic, and an ability to set and maintain personal boundaries.
•Demonstrated ability to work on multiple projects simultaneously, and to meet deadlines.
•Demonstrated knowledge of community resources
•Must be flexible, easily to adapt to changing priorities throughout the day.
•Must be highly organized and can work within multiple databases at a time.
•Reliable transportation: valid driver's license, and car insurance as required by law, as appropriate.
•Bilingual in English/Spanish preferred but not required.
•Ability to multitask and work within a fast-paced environment.
•Ability to effectively utilize computers, various software programs and desk/ cell phones technologies.
•Member of the target population a plus
SALARY
$45,000-$55,000/annually
