POSITION SUMMARY
Provides care and services for adults with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care. Services provided are primarily in the community and client’s living environment. Services may include comprehensive assessment and treatment planning, solution focused therapeutic interventions, close collaboration with clinical team to determine and guide service provision. Position will lead a team to navigate and meet the comprehensive needs of the client.
ESSENTIAL RESPONSIBILITIES
Provide regular and consistent services and supports to clients identified appropriate for community supports.
Conduct comprehensive assessments of adult clients to determine their mental health needs and develop appropriate treatment plans.
Provide short term therapeutic interventions as appropriate
Management of clinical treatment including crisis assessment and support as needed
Work closely with other members of the Community Care Team to insure client’s integrated and holistic care needs are met
Service provision shall be conducted primarily in the client’s living and other environments as necessary or appropriate
Trauma informed service delivery approach
Provide outreach and engagement when needed
Closely collaborate with other care teams within Polara to coordinate and meet client needs (Inpatient Hospital, Crisis Stabilization Unit, Outpatient Services, Primary Care)
Attend Adult Recovery Team Meetings (ARTs)
Participates as a team member and attends scheduled staffings and supervision.
Partners with client and team to identify treatment goals
Maintains clinical record per agency policy and procedure.
Attends all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainings
Management of all aspects of treatment, including referrals to additional resources when appropriate.
Performs other duties as assigned.
QUALIFICATIONS
21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member’s scope of practice; or minimum 18 years old for select administrative positions
Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
Must possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization
Master’s degree in related field (counseling, social work) licensed or license eligible preferred
2-4 years of experience working within behavioral health
Experience working with individuals in crisis situations and/or in need of intensive support services preferred
Must possess valid driver’s license
Ability to assess situations and demonstrate critical thinking and problem solving abilities
Ability to delegate and lead clinical team as appropriate
Understanding of DSM 5
