Job Description
We are offering a $4,000 Sign-on Bonus (to be paid throughout the first year of employment).

Lutheran SeniorLife provides a comprehensive array of community based health and social service programs throughout western Pennsylvania.

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Assists in the provision of food service and nutritional programs within the LIFE Center. Completes nutritional assessments, develops care plans and assures participant’s needs are met through high quality nutritional meals and services. Assists with the management of the Nutritional Services Department in the absence of the director of Nutritional Services.

Takes part in assigned participant’s assessment and reassessment, including initial and periodic, and the participant’s plan of care. Is knowledgeable about participant’s overall need and views the participant in a holistic manner.
Assesses assigned participant's nutritional status and needs, develops nutritional goals and appropriate means to meet these goals.
Participates in the development of Interdisciplinary Care Plans and attends care conferences as scheduled.
Provides nutritional care services, including nutritional assessment and counseling to participants and/or family members or caregivers as assigned.
Documents nutritional care in the medical record in accordance with center policies and accepted professional practices standards.
Documents changes in a participant’s condition in the medical record according to policy and procedures.
Assist in ensuring that participant meals are nutritious, palatable, attractive and meet participant nutritional needs.
Develops, implements and monitors provisions of therapeutic/general nutritional care to assigned participants by means of meal rounds and nourishment delivery follow-up.
Assists in departmental quality assurance measures.
Assists in the development and evaluation of policies and procedures related to nutritional care and food service.
Assists in monitoring food storage, transport and service in the center to in compliance with state health standards/guidelines.
Provides and documents in-service education for center staff as assigned.
Attends self-development programs to keep abreast of professional standards and federal, state and local regulations.
Works as a team member in cooperation with all other disciplines for the ultimate goal of excellent participant care.
Adheres to administrative and departmental policies and procedures; including, but not limited to safety, health and confidentiality.
Participates in interdisciplinary team meetings and develops plans of care for each participant.
TRAINING: On an annual basis employee will have completed training including:
the need of the participants in the center’s targeted population,
body mechanics/transfer techniques,
voluntary reporting requirements involving abuse, neglect and exploitation,
positive approach methods to manage behavior,
and two (2) sessions, (totaling a minimum of eight (8) hours), targeted at enhancing the quality of care given and the employees job performance.

Four year degree from college or university; and one (1) to two (2) years related experience and/or training in a long-term care setting preferred.
Must be “Registered” or Registered Dietitian eligible and a Licensed Dietitian in Pennsylvania or Licensed Dietitian in Pennsylvania eligible.
At least one year experience with a frail or elderly population.
Current registration with themAcademy of Nutrition and Dietetics.
Working knowledge of computers, Microsoft Word and Excel required.
