*Responsibilities*
* Provide direct patient care under the supervision of a registered nurse or physician.
* Administer medications and treatments as prescribed by the healthcare provider.
* Monitor patients' vital signs and report any changes to the healthcare team.
* Assist with medical procedures such as tube feeding and wound care.
* Maintain accurate and detailed medical documentation.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.

*Experience*
* Must be a licensed practical nurse (LPN) in good standing.
* Must have 2+ years of experience in Long Term Care.
* Strong communication and interpersonal skills.
* Ability to work effectively in a fast-paced environment.
* Proficient in medical documentation and record-keeping.
