Compensation: Pay rate will be determined based on level of experience.
Responsibilities:
Under the supervision of the Executive Director, or designee, perform diversified duties related to ongoing review of activities related to utilization of resources for the provision of a Care Management Program, including transitions of care, chronic and complex care management services, delegated case management, integration with and support of patient centered medical homes, and support of chronic disease management programs for high risk and at risk populations. Coordinate provision of evidence-based, cost-effective case management services related to all aspects of population health management. Work with Executive Director to develop and maintain an evidence-based, highly effective care management team and practice model; maintain strong relationships with Network physicians. Provide on-going direction and supervision of personnel assigned to the Southcoast care management populations. Maintain a patient caseload as needed to adequately staff the department. Will oversee new OneCare and SCO delegation. Will build program, standard work and team in collaboration with the Executive Director.
