**Available to regular status employees who are scheduled to work a minimum of 20 hours.

Responsibilities:
Under direction of the Team Leader, Care Coordination or designee, perform diversified duties including but not limited perform assessments of patients being referred to Home Care Services by the Case Coordinator; evaluate skilled services for home care within framework of health insurance reimbursement guidelines; investigate availability of resources and provide needed care, etc. Assist with education orientation of staff in regard to discharge planning and acceptance of patient issues as appropriate. Participate in all transition care initiatives to facilitate a safe discharge plan and prevent readmissions.
Qualifications:
- Graduation from an accredited school of practical nursing or equivalent is required.
- Current LPN Licensure in the Commonwealth of Massachusetts is required.
- A minimum of five (5) years related work experience is required.
- A minimum of three (3) years current nursing experience in home health care is preferred.
- Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hire
Pay Range:
USD $54,932.80 - USD $92,352.00 /Yr.
Rotation Schedule Requirement :
every other weekend and holiday
