Position Summary
- Under the general supervision of the Medical Director, the Licensed Clinical Social Worker provides clinical support to patients and their families.
- Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems.
- Being a Licensed Clinical Social Worker coaches’ patient on how to manage ongoing health conditions and provides therapy to help patients make positive behavior changes.
Required Qualifications
· Master's degree in social work from an accredited institution or a bachelor’s degree in psychology, sociology, or other field related to social work required and working toward master’s in social work from an accredited school.
Certification as a Licensed Clinical Social Worker (LCSW) is required.
· Demonstrates one (1) or more years of recent social work experience, preferably in a similar health care setting.
· Maintains current CPR Certification from the American Heart Association (Healthcare Provider) required or other approved educational programs.
· Current Arizona Department of Public Safety fingerprint clearance card (new hires must also complete the application process for this card before being released to their department).
· Ability to establish and maintain effective working relationships with all levels of MGRMC personnel.
· Ability to evaluate patients for appropriate community service needs or placement.
· Ability to effectively communicate in oral and written form required.
· Ability to establish and maintain positive communication with patients and family under stressful situations.
· An understanding of computer systems, process design and data management.
· Required to have the mental and physical ability to perform, with or without reasonable accommodation, the essential functions of the job.
· Ability to lift, carry, pull, & push 50 pounds.
Preferred Qualifications
· Knowledge of coding process
· Five years’ professional clinical experience.
· Working knowledge of Meditech system or equivalent and or ability to learn.
Principle Clinical/Technical Duties
· Category: Patent Analysis
o Conducts and completes a social work psychosocial assessment of patient family within appropriate time frame as requested.
o Actively participates in case conference to identify patient’s discharge planning needs as requested.
o Acts as consultant to MGRMC Case Managers of all disciplines and other departments within the hospital as needed.
o Compare the standard case management plan against the patient’s individual needs in such areas as social and economic data, age-specific criteria, family resources, functional abilities, knowledge needs, potential risk factors, and complications and special issues.
o Provide counseling support for patients and family within hospital owned clinic as needed.
Category: Documentation
o Documents in the medical record social services involvement and recommendations.
o Document patients’ status and variances, working toward achievement of patients’ goals and expected outcomes according to the disciplinary plan care.
o Provides concurrent chart review to ensure evidence-based practice for core measures are implemented.
Category: Communication
o Collaborates with physicians and other health care providers for appropriate orders and status utilizing the MCG.
o Will collaborate with finance to review any insurance denials and assist with needed documentation to re-file claims.
o Plans, participates in, and follows through with health care team meetings, as needed.
· Category: Clinical Knowledge
o Identifies the anticipated DRG, length of stay, and discharge or transfer date.
o Oversees Swing bed program. Serves on hospital committees as requested.
o Carries out identified quality improvement initiatives.

o Pursues professional education to maintain current clinical competence in a chosen field.
