Job Title: Clinical Manager

Job Summary: The Clinical Manager provides patient centered nursing care in an outpatient clinic according to established scope of practice guidelines and standards of care. Develops, coordinates, and implements the patient plan of care in collaboration with the health care team.

The Clinical Manager is a strong patient advocate and liaison that assists the Executive Director and Medical and Dental Directors in ensuring a consistent level of high-quality patient care through provider and staff engagement and through direct patient care. The Clinical Manager works collaboratively with the Front Desk Manager, Program Assistant and other key staff/volunteers.

The Clinical Manager is responsible for both onsite and mobile medical services. All duties on behalf of Park Street Healthshare, Inc (PSH) under the direction of the Executive Director and in accordance with the organization’s policies and procedures.

Responsibilities

Including but not limited to the following:

·        Assist the Executive Director and Medical and Dental Directors with clinical provider staffing, recruitment, onboarding, clinical policy reviews and implementation, and other duties as requested.

·        Ensures adequate staffing, supplies and equipment for both brick & mortar and mobile medical clinics.

·        Creates and ensures proper protocol and training for patient care including POC testing, referrals and follow-up.

·        Ensure that all lab and DI results are placed in patient charts for review by the medical providers. Notify the ordering provider of abnormal results. Notify ordering provider as well as the Medical or Dental Director of all stat results.

·        Direct, oversee and manage the immunization program.

·        Support and encourage our immunization program by promoting vaccinations at all clinics. This should include reviewing patient immunization records and providing education/resources to our patients as needed in this effort.

·        Work collaboratively with UVM Bridges to Health, Rutland Welcomes, Bridge to Rutland and other programs to serve our migrant and non-English speaking patients.

·        Identify services and or potential partnerships for health-related services such as eye care, audiology tests, nutritional counseling, mental health counseling, etc.

·        Prepare Clinic information for Board presentations as requested by the Executive Director.

·        Engage with non-clinical volunteers, interns and other individuals seeking to learn in the Free Clinic environment.

Qualifications

·        Minimum of 3 years full time or 5 years part time experience working as an RN in a clinical environment

·        Current State of Vermont nursing license without stipulations

·        Ability to pass a background check.

·        Willingness to take initiative for new responsibilities and projects.

·        Understands and complies with HIPAA regulations and patient confidentiality.

·        Strong interpersonal, team, organization, time management and prioritization skills

·        Ability to work well under pressure and handle difficult situations and people in a calm and respectful manner.

·        Proficient at Microsoft Office Suite

·        Must be able to lift up to 25 lbs.
