    Job Details
  • A hospital in Arizona is looking for experienced ICU nurse for a travel assignment.
  • The right candidate will have 2 years of experience, and hold an active BLS, ACLS and AZ license or compact.
  • Must pass a dysrhythmia exam upon acceptance (2 max attempts) and there is also a pass/fail Sim Lab on day 1 of orientation.
  • COVID vaccine is required or declination form with supporting documentation from clergy or MD.
    Job Requirements
  • 2 years of experience required.
  • Active BLS certification required.
  • Active ACLS certification required.
  • Active AZ license or compact required.
  • Must pass a dysrhythmia exam upon acceptance (2 max attempts).
  • Must pass a pass/fail Sim Lab on day 1 of orientation.
    Schedule Information
  • Every other weekend required.
  • Could be every 3rd weekend.
  • Work into permanent staff rotation for holidays.
    Unit Specific Information
  • Ratios: 1:2 ICU & 1:3 MS/Tele
  • Weekend Requirement: Every other weekend. Could be every 3rd.
  • Scrub Color: Navy.
  • Tele: GE Monitors. Must be able to interpret EKGs.
  • Vents: Yes
  • Hospitalist or Intensivist in house: Intensivist 24/7
  • IV Team: No
  • RT 24/7: Yes
  • Pharmacy 24/7: Yes
  • Floating: First to Float to tele or the monitor room
  • Floor specific orientation; 2 shifts on the floor.
    Additional Details
  • ACLS, BLS; Auto-offer based off of Clinical Prescreen.
  • Dysrhythmia Exam through UNA is required 24-48hrs prior to signing contract. MAX ATTEMPTS IS 2 - NO EXCEPTIONS, NO 3rd ATTEMPT ALLOWED FOR ANY REASON.
  • If they do not pass in 2 attempts, they will be a DNU for CSH AZ and CSH Sacramento for 6 months.
  • There is a PASS/FAIL Sim Lab on day 1 of orientation. If they fail they will be sent home and made a DNU for 6 months.
