*Job Summary*

As a Hospice Nurse, you will be responsible for providing compassionate end-of-life care to patients in hospice settings. You will work closely with patients, families, and interdisciplinary teams to ensure comfort and quality of life during this sensitive time.

*Responsibilities*
* Conduct patient assessments and develop individualized care plans
* Administer medications and treatments as prescribed
* Provide emotional support and guidance to patients and their families
* Monitor patient conditions and report changes to the healthcare team
* Collaborate with physicians, social workers, and other healthcare professionals
* Educate patients and families on end-of-life care options
* Maintain accurate medical records and documentation

*Qualifications*
* Strong background in patient care with experience in hospice or palliative care preferred
* Proficiency in MDS documentation and sterile processing techniques
* Ability to perform triage assessments effectively
* Knowledge of CT procedures, memory care practices, and ostomy care
* Familiarity with medical records management systems such as Cerner
* Experience working in a medical office setting is advantageous

Job Type: Contract

Pay: $48.00 - $52.00 per hour
