*Responsibilities:*

As a Registered Nurse Home Health, you will:
* Align activities with Elara Caring’s vision and leadership goals.
* Maintain patient and staff privacy as per HIPAA regulations.
* Assess patient needs and develop comprehensive care plans.
* Assume primary responsibility for caseload, coordinating and evaluating treatment plans.
* Communicate significant findings and changes in patient condition to the Clinical Supervisor and healthcare team.

*Requirements:*
* *State RN License:* Current and unrestricted.
* Home health experience & Oasis experience strongly preferred
* *Experience:* Minimum of 1 year in a clinical care setting.
* *Travel:* Able and willing to travel within the branch/office coverage area.
* *Vehicle:* Must have a dependable vehicle, valid driver’s license, and current auto insurance.
* *Education:* Associates Degree or bachelor’s degree in nursing preferred.
