Our client is looking for a caring and experienced Case Coordinator to join their team!
Title: Case Coordinator
Type: Contract 3+ months
Location: Hybrid in Cherry Hill, NJ
Hours: Monday-Friday, 8am to 5pm
Responsibilities:
1. Manage aspects related to assigned caseload that pertain to access to Bancroft services, family advocacy, and funding and ensure cases are approached in a thoughtful, holistic manner that reflects Bancroft’s core values.
2. Schedule and coordinate the annual plan meetings and ensure that all care team members are included.
3. Ensure that all documentation for the annual plan is promptly prepared and provided to the appropriate parties.
4. Ensures that the annual plan accurately captures needed services and ensures that funding authorizations are in place for the services.
5. As needed facilitates eligibility and benefit checks and oversees the process of re-determining and/or restoring eligibility for benefits and entitlements.
6. Assist with securing funding authorizations for durable medical equipment and adaptive devices.
7. Support the process when individuals transfer services to a new program or service line by ensuring that transfer meetings are held with external care team members’ input and that all documentation is in place for no disruption in funding.
8. Ensure that all pertinent contact information for each individual and his/her care team and relationships is accurately maintained in the electronic database (Salesforce). Enters discharges and transfers into Salesforce.
9. Oversee the electronic files for assigned caseload.
10. Ensure that guardianship and Power of Attorney documentation is on file.
11. Assist families, individuals served and residential care teams with resolving medical insurance issues.
12. For new admissions, attend the pre-admission and admissions meetings to meet the external care team members and to learn about the service needs of the individual being admitted.
13. Coordinate the discharge planning process. Prepare the discharge letter and the discharge summary.
14. Maintain a clear understanding of the funding regulations relevant to assigned caseload.
Qualifications:
- Bachelor's degree in Human Services or Related field
- Minimum of two years’ experience of related experience in case management and family advocacy required.
- Nurse Case Manager or Social Work background.
