We are looking for a full-time, on-site Licensed Clinician to assess, authorize, and complete individual rehabilitation plans for adults with serious mental illness.
•Performs a comprehensive rehabilitation needs assessment.
•Contributes to the development, implementation, monitoring, and updating of the individualized rehabilitation plan.
•Provides clinical support and on-call coverage to staff and consumers for crisis intervention and emergencies.
•Support consumers who may be experiencing symptoms related to mental illness and/or substance use.
•Maintains compliance with all regulatory, contractual, accreditation, and quality standards including the Agency Performance Management Plan, policies and procedures, and corporate compliance standards.
•Follows Agency financial procedures as it relates to the submission of individualized rehabilitation plans.
•Documents service delivery accurately and timely while ensuring consumer confidentiality.
•Organizes, attends, and participates in team, supervision, and training meetings.
•Promotes wellness and recovery practices, a consumer choice focus, and valued life roles for each individual served.
Qualifications:
•Master's degree in non-profit or human services management, mental health, substance abuse, or a related discipline.
•Knowledge of mental illness, addiction, and/or homelessness required.
•Must possess a current NJ Licensed Clinical Social Worker, NJ Licensed Professional Counselor, or NJ Registered Nurse.
•Must possess a valid driver's license, with a good driving record.
