Job Description - Perform advanced clinical pharmacy duties to optimize pharmacotherapy for transplant patients.
- Conduct proactive and reactive medication regimen reviews to ensure safety and effectiveness.
- Collaborate with healthcare professionals to manage patient drug therapies and improve care transitions.
- Serve as a mentor and preceptor for pharmacy students and residents.
- Participate in quality improvement initiatives, including formulary management and medication use evaluations.
- Support innovative research and contribute to publications and presentations.
Qualifications - Education : PharmD from an accredited institution.
- Experience :
- Completion of ASHP-accredited PGY1 residency (required).
- PGY2 residency in solid organ transplant or equivalent experience (preferred).
- Licensure : Eligible for Texas pharmacist licensure.
- Skills : Strong analytical and collaborative abilities, with a passion for patient-centered transplant care.
