Title: LTSS Service Care Manager
Duration: 06 Months (possibility of extension)
Location: member facing (Source from Corpus Christi)
Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):

• LVN needed to assist with follow up calls for members.
• LVN will work assigned tasks
• LVN may perform field visits in Corpus Christi area if needed

LVN will fill put required forms for each assessment
