A federally qualified health center located in the Bronx is actively seeking a dedicated and diligent professional to join their staff as their new Case Manager. In this role, the Case Manager will contribute to a program culture that is person-centered, holistic, focused on positive youth development, and committed to continuous improvement.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Temp to perm
- Schedule: Monday to Friday
- Hours: 9am to 5pm (1-hr lunch; unpaid)
- Patient Population: Ages 18 to 24
Responsibilities:
- Provide integrated counseling and case management services that effectively engage young adults and promote personal growth and wellness, housing stability and accomplishment of service plan goals on an ongoing basis
- assess strengths and challenges and develops an individual service plan that reflects the youth's progress and stated goals
- Meet with youth regularly to determine progress with goals
- Advocate on behalf of youth with external stakeholders
- Collaborate with and make needed referrals to community providers
- Create and maintain confidential records, charts and logs for youth documenting all relevant information from individual and group interventions as well as with collateral services
- Ensure that youths' files are compliant with all regulatory and other requirements
- De-escalate crises and attend to emergencies following agency protocol
- Assist with property management issues in reporting to landlords and ensuring work orders are complete
- Perform other duties, as needed
Qualifications:
- 4+ years of experience in Case Management, Social Services, Youth Development, and/or a related field
- High School Diploma / GED
- Demonstrated knowledge of and experience working directly with young people through approaches that emphasize resiliency, strength based youth development, and trauma informed care
- Experience working with diverse groups
Desired Skills:
- Bachelor's Degree in Psychology, Social Work, and/or a related field
- Experience utilizing evidence-based and other social service models
