Elara Caring provides in-home healthcare services and is seeking an experienced Occupational Therapist to evaluate patients, develop treatment plans, and provide therapy to enable patients to regain independence in daily living activities. This role will involve collaborating with a team to deliver compassionate, high-quality care aligned with the company's mission.

Key Responsibilities:
* Conduct occupational therapy evaluations to determine patients' rehabilitation needs and potential
* Develop customized treatment plans tailored to each patient's goals
* Provide direct occupational therapy to patients in their homes to improve functional abilities
* Educate patients and caregivers on using adaptive equipment and integrating therapy into daily routines
* Communicate with supervisors, physicians, and care team on patient progress and changes
* Verify and update treatment plans per physicians' orders and patients' evolving needs
* Ensure therapy provision aligns with standards of practice, regulations, and care plan
* Supervise and direct Certified OT Assistants, Home Health Aides, and other staff

Requirements:
* Graduate degree in Occupational Therapy
* Active state licensure as an Occupational Therapist
* 1+ years experience as an OT in a clinical setting
* Strong communication and documentation skills
* Ability to travel to patients' homes within service area

Pay: $95,000.00 - $105,000.00 per year
