*MedAdventures *is looking for a skilled and Physical *Therapy Assistant* to join one of our Skilled Nursing Facility partners in the *Milford, DE*

This is a 13-26 week contract role that can convert full time upon completion of assignment!

*Compensation and Schedule:*
* Target Pay Range $35-40/HR W2

*Responsibilities*
- Assist in the development and adjustment of patient care plans
- Provide hands-on therapy to patients as directed by the physical therapist
- Educate patients and their families on exercises and activities
- Document patient progress and report back to the supervising physical therapist
- Assist in discharge planning and transitioning patients to home care or other facilities

*Requirements*
- Knowledge of anatomy, physiology, and trigger point therapy
- Experience or interest in acute care, orthopedics, and hospital settings
- Ability to provide compassionate patient care
- Strong communication skills for interacting with patients and healthcare professionals
- Understanding of discharge planning process
