· Number of Staff – 15 RNs, 15 SW
· Type of staff:
o Day Shift – 18 staff members/skill mix
o Night Shift - NA
· Patient Ratios – 1: 30 RN to patient
· Type of equipment – Computer, office equipment
· EMR – Meditech / Midas
Must have: candidates without these skills will not be considered for the role.
· Meditech or HCA experience
· Bed side case management experience at least 1 year
· Post acute care knowledge
Preferred or nice to have: candidates with these skills will be considered first.
· Meditech/Midas or HCA experience
· Throughput experience
List typical procedures performed on unit(s):
· Discharge planning and progression
· Hard conversations with family, patients and open discussion with MD for plans of care
