*Job Summary*
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including long-term care facilities, primary care, and hospice environments. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive high-quality care tailored to their individual needs.

*Duties*
* Administer medications and treatments as prescribed by healthcare providers.
* Monitor and record vital signs, including blood pressure, temperature, and pulse.
* Assist in health coaching and education for patients and their families regarding health management.
* Maintain accurate medical documentation of patient care and progress.
* Provide basic life support as needed in emergency situations.
* Collaborate with healthcare teams to develop and implement patient care plans.
* Support family planning initiatives by providing relevant information and resources.
* Participate in hospice care activities, ensuring comfort and dignity for terminally ill patients.
* Assist with internal medicine procedures under the guidance of supervising physicians.

*Requirements*
* Valid Licensed Practical Nurse (LPN) license in the state of practice.
* Experience in long-term care or hospice care preferred.
* Proficient in taking vital signs and understanding medical systems.
* Strong skills in medical documentation and patient assessment.
* Basic Life Support (BLS) certification required.
* Excellent communication skills with a focus on patient-centered care.
* Ability to work collaboratively within a healthcare team while demonstrating empathy and professionalism.
* Knowledge of family planning practices is a plus.
