*Position Summary:*

Our Client is seeking an RN Case Manager. An RN Case Manager is a registered nurse who coordinates and manages patient care to ensure that individuals receive appropriate healthcare services. They assess patients' needs, create care plans, facilitate communication between healthcare providers, and advocate for patients’ health needs. Their role often involves working with patients with chronic conditions, complex medical histories, or those transitioning between different care settings (e.g., from hospital to home). The goal is to improve patient outcomes while optimizing healthcare resources and reducing costs.



*Key Responsibilities:*

* *Assessing patient needs*: Evaluating medical, psychological, and social needs.
* *Creating care plans*: Developing individualized plans to meet patient health goals.
* *Coordinating care*: Communicating with healthcare teams and arranging services.
* *Monitoring progress*: Tracking patient outcomes and adjusting care plans as needed.
* *Advocating for patients*: Ensuring patients receive necessary services and resources.
* *Providing education*: Educating patients and families about care and treatment options.
* *Ensuring smooth transitions*: Managing transitions between care settings (hospital, home, rehab).

*Required Qualifications:*

Active, unrestricted certification/licensure as an RN in California State.



*Certifications Required:*

ACLS, BLS REQUIRED
