CLINICAL HOSPITAL PHARMACIST JOB DESCRIPTION: The Clinical Pharmacist position is responsible for providing clinical and operational pharmacy services to the highest standard of pharmaceutical care principles for each patient within their area of responsibility in an acute hospital setting. The position is responsible for therapeutic assessment, interventions, and plans for individual patients as well as patient populations. The Pharmacist is a valued member of the Interdisciplinary Care Team and assumes responsibility for medication-related outcomes. The pharmacist aligns with the pharmacy enterprise’s vision of improving outcomes and bringing value to our patients. SHIFT/SETTING: Location: Pittsfield, MA Schedule: Monday-Friday ( 7a-3.30p and/or 8ap and/or 8.30a-5p). Must be available for any/all shifts as needed Setting: Oncology Start Date: 2/24/25 CLINICAL HOSPITAL PHARMACIST RESPONSIBILITIES: - Assessing patient therapy for appropriateness, incorporating age specific modifications, and clinical interventions to prevent adverse drug events. -They will also conduct clinical research and other professional development programs. -He/she will serve as a liaison to other departments at the Cancer and Infusion Center by providing operational support and educational services for new treatment regimens. - The Staff Pharmacist in Oncology demonstrates in-depth knowledge of the regulations governing the preparation of intravenous admixtures, chemotherapy, and other sterile products and supervises Pharmacy technicians and other support personnel. - Provides oversight and participates in maintaining and storing adequate inventory of all medication required for the practice. - Develop and update department and nursing policies. - Work with the Oncology team to standardize ordering procedures for chemotherapeutic medications. CLINICAL HOSPITAL PHARMACIST REQUIREMENTS: - 5+ years extensive acute care clinical experience - REQUIRED - Unrestricted Massachusetts PharmD License - REQUIRED - BLS/ACLS - REQUIRED - Completion of ASHP accredited PGY1 Pharmacy Residency in Oncology - STRONGLY PREFERRED - Board Certification Oncology Pharmacist (BCOP) Certification is required - STRONGLY PREFERRED
