    Job Requirements
  • ARRT Certification
  • Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration
  • Proof of vaccine series or completion and approval of client medical or religious exemption form (Preferred, not required)
  • Positive MMR titers ONLY from within the last 15 years
  • Completed within 1 year of start updated annually
  • Proof of full Hepatitis B series or at least two vaccines from the series completed or signed declination
  • Positive Varicella titers ONLY from within the last 15 years
  • Proof of vaccination within past 10 years, declinations not accepted
  • 2-Step TB test with negative reads within the last 6 months
    Schedule Information
  • (5) 8 HOUR SHIFTS
  • NO WEEKENDS
  • NO CALL
  • NO HOLIDAY
    Additional Details
  • Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration. Attestations not allowed. Needs to be rerun every 2 years with RN license renewal & upon expiration. Temporary Licenses are NOT accepted – must have a valid permanent license.
