Job Description

Job Title: LTSS Service Care Manager

Location: Remote with Member Facing in Sarasota/Collier/Lee, (County) Florida

Mon-Fri 8A-5P


Day-to-Day Responsibilities:

  • Manage a case load of healthcare members with long-term care needs.
  • Monthly and quarterly member contact, including 80% travel. Remote role requiring a driver’s license.
  • Complete member assessments with caregivers or providers to gather information for care plan development.
  • Monitor service delivery, ensuring that services are provided without gaps. Follow-up with members, caregivers, or providers through in-person visits and telephonic contact.
  • Authorize and coordinate service referrals, ensuring functional deficiencies are identified and addressed in care plans.
  • Assist in coordinating the development of informal or voluntary services within the member care plan.
  • Collaborate with discharge planners, physicians, and other parties to ensure a proper discharge plan, care plan, and coordination of services.
  • Assist members in filing and resolving complaints and appeals.


Required Skills/Experience:

  • 2+ years of Care Management experience
  • Experience with electronic medical health records
  • Bilingual (Spanish)


Preferred Skills/Experience:

  • Home health experience
  • Long-term care experience
  • Discharge planning experience
  • Proficiency in Microsoft Office


Education Requirements:

  • Bachelor’s degree
  • 2-4 years of related experience
